YOUR FIRST VISIT

We will help make your first visit the best ever. Comfort and relaxation are the words I want to come to your mind when you visit my office, so if you need a little extra support for your first visit, we welcome any friend or family member you wish to bring.

Your initial appointment will begin with REGISTRATION. To save time, much of this can be done before you arrive. Please assist us by providing the following information before or at the time of your consultation:

  • Your completed medical history and registration forms (you may save time by downloading and completing at home before arriving). We have to know about allergies, sensitivity to anesthetics, long-term medications--your whole health picture.

  • If you have medical or dental insurance, bring the necessary completed forms and your card so that we may make a copy for your records. This will save time and allow us to help you process any claims.

  • If your old dentist or physician has taken x-rays, you may request that they forward them to our office with your old records, but while this may be helpful it is not absolutely necessary.

IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

We will then begin the EXAMINATION. We'll need an hour or so to examine your teeth, gums, jaw alignment, and soft tissues, and to perform an oral cancer screening. We'll take x-rays to find areas of infection that need treatment, and, if necessary, impressions will be made for hard cast models to examine your bite.

Your initial appointment will also consist of a CONSULTATION explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
